Quality of Life Resource and Referral LLC, helps children and adults as well as families with eligible individuals with intellectual and/or developmental disabilities (IDD), serious mental illness (SMI) that includes severe and persistent mental illness (SPMI) and co-occurring disorders. This may include a primary substance use disorder diagnosis. gain greater independence by learning and practicing daily living skills. Our team of Therapist, Direct Service Providers, Peer Support Specialist, and Qualified Professionals help those we support build and retain the skills needed to independently live at home and when they go out into the community.
Through various programs, individuals can receive one-on-one support based on a personalized plan to address their specific needs. We also help families learn more about community resources in addition to providing periodic breaks of relief for primary caregivers.

Our Community Living and Support Services offer a variety of Innovation Waivers including Medicaid and Private Insurance Services. For more information on how we can individually support you and your family, please reach out to us or view our list of programs.
Individual and Transitional Supports
Individual and Transitional Supports a home and community-based service. It offers an option to innovations for eligible individuals with intellectual and/or developmental disabilities (IDD), serious mental illness (SMI) that includes severe and persistent mental illness (SPMI) and co-occurring disorders. This may include a primary substance use disorder diagnosis. to continue living in their community with the choice of other service options and meaningful day activities.
The Individual and Community Services team supports individuals with intellectual and/or developmental disabilities (IDD). We provide one-to-one services uniquely tailored to help each unique person acquire and maintain daily living skills and community support that is needed in todays world.
The Individual and Transitional Services team supports individuals with intellectual and/or developmental disabilities (IDD), serious mental illness (SMI) that includes severe and persistent mental illness (SPMI), and co-occurring disorders. This may include a primary substance use disorder diagnosis, we provide one-to-one services uniquely tailored to help each unique person acquire and maintain daily living skills. Individual and Transitional Services enable individuals to increase or maintain their opportunities for independence. We also help individuals develop social skills that are valued by non-disabled members of the community.
Respite
Quality of Life Resource and Referral LLC, provides Respite (periodic/scheduled support) and temporary relief to primary caregivers from the responsibility and stress of caring for an individual with intellectual and/or developmental disabilities, serious mental illness (SMI) that includes severe and persistent mental illness (SPMI), and co-occurring disorders. This may include a primary substance use disorder diagnosis. Our Respite service enables primary caregivers the opportunity to participate in planned or emergency events. It also gives primary caregivers a much-needed break for their own self-care or spending much needed time with friends and family Likewise, our Respite services are also available to individuals who request a break and time away from theirprimary caregiver.
Eligibility
1915i Children and adults with intellectual and/or developmental disabilities (IDD) who receive the
Innovations Waiver, serious mental illness (SMI) that includes severe and persistent mental illness (SPMI)
and co-occurring disorders. This may include a primary substance use disorder diagnosis.

1915i Children and adults with the appropriate assessments to determine eligibility for the Innovations Waiver
serious mental illness (SMI) that includes severe and persistent mental illness (SPMI) and co-occurring
disorders. This may include a primary substance use disorder diagnosis.
Eligibility For Most Services
Most Have one of the following before you can begin services:
Psychological evaluation (within the last 3 years for a child, within the last 5 years for an adult)
Qualifying diagnosis outlined by the psychological evaluation
SNAP Assessment
